Rouge Valley health invites patients to learn about a new program to facilitate patients transitioning home after hospital time

Rouge Valley Health has produced a You-Tube video that highlights a new specialized program focused on assisting patients who are transitioning from hospital to home. Director of Public Affairs and Community Relations, David Brazeau, says the three minute video is a great educational tool for those considering the program, which is aimed at helping patients, primarily seniors, to recover their strength and functioning. The Transitional Restorative Care Program is currently available at Rouge Valley’s Ajax-Pickering hospital. To see the video, click here.
